Information procedures

 

 

When visiting our offices, follow the procedure below in order to obtain quick information.

 

1.        Reception or Classification: Feeling out service request form, which will be presented to the cashier and     the Classifier will assist you in any concern.

 

2.        Cashier or Collection Section: Once the service requested is paid for, the cashier will indicate you the Department to be addressed.

 

3.        Department of Pertaining Service: Here you will deliver the documentation as required, and will wait your turn in order received or by a number if so would be case.

Translation Unit .

Documents originated in the United States:

This documents should be notarized, and the Notary Public’s signature certified by the County Clerk.

 

Documents originated in the Dominican Republic: Birth and Marriage Certificates, Divorce Acts, and Judgments should be legalized by the Central City Clerk’s Office (Central Board of Elections), by the Secretariat of State of Foreign Affairs, and certified by the United States Consulate in the Dominican Republic.

 

School records, diplomas, school certificates: This documents should be certified by the Secretariat of State of Education, and Culture, as the case should be, and legalized by the Secretariat of State of Foreign Affairs in the Dominican Republic

Legal Department.-

Documents originated in the United States to be used in the Dominican Republic:  

Funeral Home documents for transportation of human remains:

  • Death certificate, the City Registrar signature should be ratified by a higher authority or Official.

  • Embalming Affidavit. This document should be signed by the embalmer, and notarized. The Notar.

  • Public’s signature certified by the Country Clerk.

  • Letter of None Contagious Disease by the Health Department, ratified by a higher authority, stating on none existent particular outbreaks or contagious disease. This documentation will be translated to Spanish and legalized, by the Consulate, as long as complying with the provided requirements.
 
Authorization to obtain a minor’s passport issuance:
  • Either one of the parents or guardian should appeared in person, with valid identification. 

  • Passport, or any other official identification.

  • Birth Certificate duly legalized (original or copy), name, identification, and address of the person being authorized to withdraw the minor’s passport in the Dominican Republic.

Authorization to collect a Retirement Check in the Dominican Republic:

The retiree should appear with the appropriate identification , passport or any other official Id), the name, identification or voter’s registration number, and address of the person being authorized to collect his/her pension’s check in the Dominican Republic.

Documents for Animals/Pets Transportation to the Dominican Republic:
  • Health certificate duly notarized, issued by the Veterinarian.

  • Notary Public’s signature certified by the County Clerk.

 

Power of Attorneys Section
In order to obtain a power of attorney, the constituent should present his/her identification card or passport, the proxy’s complete name, identification number, and address. If it about properties, bank accounts or vehicles, the information on District Real Estate Record, location, the bank name and address, account number or certificate of deposit number, and the vehicle’s general information, respectively.

Passports Department:

 

Passport Renewal: Every two years, from the issuing date, to a maximum of six years.

 Renewal for two years requires the passport and US$65.00. For 4 years $113.50

Change of passport’s book: Six years after the issuing date. Requires expired book. The Cost is US$120.50

 First time issuance or by loss: Requires an official identification and voter’s registration card, birth certificate, legalized. The Cost is US$120.00 for six years, US$100.50 for 4 years and US$80.50 for 2 years 

Change of marital status: Either the marriage certificate or divorce certificate should be presented, as the case would be. The cost is for US marriage $38.00 for Dominican marriage $12.00

VISA SECTION

In order to obtain a visa to visit the Dominican Republic, you require a valid passport, and a petition letter  when not requested in person.

 

Students should present a proof of admission from the University in which they have been registered.

Most of the visas should be approved by the Secretariat of State of Foreign Affairs in Santo Domingo.

 

The following is a list of countries whose Citizens do not required a visa, neither a tourist card (90 days stay), to enter the Dominican Republic (only a valid passport):
